In-Home Postpartum Care: Doula vs. RN/LPN

Not sure what kind of in-home care you need? Here’s a side-by-side look at the support a Postpartum Doula provides compared to a Registered Nurse (RN) or Licensed Practical Nurse (LPN).

Postpartum Doula

  • Role: Supportive presence focused on easing the transition into parenthood.
  • In-Home Services:
    • Newborn care guidance (diapering, soothing, sleep basics)
    • Emotional support for parents
    • Light household help (baby laundry, bottle/pump sanitation, tidying)
    • Education on newborn cues, bonding, and safe sleep
    • Referrals to lactation consultants, therapists, or pediatricians
  • Limitations: Non-medical; cannot perform clinical care or administer medications.
  • Best Fit: Families wanting holistic, non-medical support.

Registered Nurse (RN) / LPN

  • Role: Nurturing support with medical expertise and clinical oversight.
  • In-Home Services:
    • Newborn monitoring (feeding, breathing patterns, temperatures)
    • Maternal care (C-section checks, vitals, recovery support)
    • Administering prescribed medications or treatments
    • Specialized feeding support (latch assistance, formula prep, tube-feeding)
    • Early recognition of maternal or infant complications
  • Advantages: Performs all in-home services plus an extra peace of mind for multiples, preterm infants, or maternal health concerns.
  • Best Fit: Families seeking nurturing care with clinical expertise.

Insurance & Coverage in Montana

Here’s what’s available now — and what’s coming soon — for families seeking postpartum care support in Montana.

12-Month Postpartum Medicaid (Now) Doula Coverage Begins Jan 1, 2026

Medicaid Coverage

  • Montana Medicaid provides 12 months of postpartum coverage for eligible mothers (effective July 2023).
  • Skilled in-home nursing (RN/LPN) may be covered if medically prescribed.
  • Doula services will be reimbursable starting January 1, 2026.
Tip: Families may need a doctor’s order and visit notes for Medicaid to cover in-home nursing.

Private Insurance

  • Most private plans do not cover doula services, but many allow HSA/FSA funds.
  • In-home RN/LPN care may be eligible under home health benefits with a doctor’s order.
  • We can provide a superbill for potential out-of-network reimbursement.
Ask your plan about “in-home postpartum nursing” or “home health services,” not “nanny” care.

Insurance & Payment FAQ

Are Willow Nurse Wellness services covered by insurance?

Most of our postpartum nursing and doula services are private-pay and not billed directly to insurance. However, there are a few important options for families:

What does this mean for me?

We recommend that families expect services to be private-pay, but ask their insurance company if postpartum or lactation support is reimbursable under their plan. Our team is happy to provide the paperwork you need to submit for possible reimbursement.
